What does the form ask for?

• Contact info for your Primary care provider and the date of your last visit
• 10-year health history, including health conditions, prescriptions, and treatments
• Family history of cardiovascular disease, polycystic kidney disease, and Huntington’s disease
• Injury/disability claims in the past 5 years
• Recent and planned international trips
• Driving violations

Remember that you, the proposed insured, must log in and fill it out.
